Registering with a GP practice

Our Practice catchment area is shown opposite.

To register permanently, you’ll need to complete a registration form:

  • Download a registration form, complete it, and then email it to  gram.ellonadministrator@nhs.scot

When you register with us, it’s recommended that you provide information to confirm your:

  • identity
  • address

This means you can be matched to your existing NHS records. You shouldn’t be delayed or refused registration at a GP practice if you can’t provide identification or proof of address.

 

Area

Registering as a Temporary Resident

Whilst it is always the preference to be seen by your own GP as they have access to your full medical records, if you are staying in the local area for up to 3 months , require medical advice  & travelling to your own registered GP is not possible you may be able to be seen as a temporary resident. Please contact 0345 337 1150 & one of the admin team will advise further. 

You and Your Doctor

Patients are registered with the Practice, not with an individual GP; you can at any time express a preference for a particular doctor, for either or all of your medical needs or on a case by case basis.  However, not all of the doctors based within the Practice provide all services , furthermore specific requested doctors may not be always be available on the day requested. 

Accessing someone else’s information

As a parent (if child aged 12+), family member or carer, & wish to discuss any aspect of their medical care, you would require written permission in the form of a practice consent form which has to be completed & signed by the patient. Please call 0345 337 1150 for further information.
